چکیده

A 49-year-old man with morbid obesity (6′5′′, 360 lbs), diabetes mellitus, hypercholesterolemia, and a history of systemic arterial hypertension had an aortic valve replacement for severe stenosis (aortic valve area, 0.9 cm2) with exertional dyspnea and left ventricular hypertrophy on echocardiogram. Immediately after operation an electrocardiogram was recorded (Figure 1). The electrocardiogram shows a regular tachycardia at a rate of 110 beats/min with a wide QRS (0.17 s duration) and a left-bundle-branch-block configuration: broad and notched or slurred R waves are present in leads aVL, V5, V6 without Q waves in these leads, and the peak of the R wave in lead V6 is prolonged to 0.07 s (1). In addition, wide QS waves in leads V1, V2 have a time from the onset of the QS to the nadir of 0.06 s and no notch on the downslope of the QS (2). No definite P waves are visible. Is this ventricular tachycardia or a supraventricular tachycardia with left bundle branch block? If the latter, what kind of supraventricular tachycardia is it? A preoperative electrocardiogram did not show left bundle branch block but did show a long P-R interval and left ventricular hypertrophy (Figure 2) (3). Although ventricular tachycardias often have QRSs resembling left bundle branch block, the morphology rarely is typical (2). Thus, the perfect left bundle branch block in this patient suggests a supraventricular tachycardia with the new onset of left bundle branch block, which could be rate related and/or due to temporary or permanent damage to the left bundle branch during removal of the patient’s heavily calcified bicuspid aortic valve and its replacement with a 29-mm St. Jude mechanical prosthesis. Sustained monomorphic left ventricular outflow tract tachycardia early after aortic valve replacement.

Sustained monomorphic ventricular tachycardia (VT) after valve surgery is uncommon. Cases of focal VT or bundle-branch re-entry after aortic valve surgery have been reported. We present the case of a 60 year-old patient with an incessant outflow tract VT early after aortic valve replacement. We suggest the disease process affecting the valve and adjacent area, and/or the surgical procedure,...

Bundle branch reentrant ventricular tachycardia after transcatheter aortic valve replacement

Introduction Bundle branch reentrant ventricular tachycardia (BBR-VT) is a well-described entity that accounts for 6% of inducible sustained monomorphic VT during electrophysiology studies. BBR-VT is a macroreentrant VT involving the HisPurkinje system and usually requires some form of conduction delay in the His-Purkinje axis. BBR-VT is commonly seen in patients with nonischemic dilated cardio...

بررسی نتایج پیگیری یک ساله بالون والولوپلاستی تنگی مادرزادی دریچه آئورت در کودکان به مدت 10 سال در بیمارستان قلب شهید رجایی

Abstract Background: Balloon valvuloplasty uses for treatment of aortic stenosis from 20 years ago. Regarding loss of information about results of balloon valvuloplasty of hereditary aortic stenosis, this study was done to determine prognosis and complication of balloon valvuloplasty during last 10 years. Methods: This study was done on existing data of all children with hereditary aortic...

Changes in the QRS complex after aortic valve replacement.

The electrocardiograms of 50 patients after isolated aortic valve replacement were examined. Two main types of postoperative QRS changes were found. In I3 patients intraventricular conduction defects developed, predominantly in theform ofa left anterior hemiblock as an isolated lesion or combined with other focal blocks. The other prominent finding was the appearance of abnormal Q waves suggest...

Figure-8 Tachycardia Confined to the Anterior Wall of the Left Atrium

Incisional atrial tachycardias have been described most frequently in patients with previous corrective surgery for congenital heart defects and mitral valve disease. Less information is available on atrial tachycardias appearing late after isolated aortic valve surgery. We report the case of a patient who developed a left figure-8 tachycardia after undergoing aortic valve replacement. During e...
